Qualifications

• Strong interpersonal and communication skills for collaboration with diverse healthcare professionals.
• Knowledge of Epic EHR and proficiency in relevant medical software.

Responsibilities

• Conduct comprehensive assessments of patients in long‑term care facilities, including medical history, physical examinations, and medication reviews.
• Develop and implement individualized care plans for long‑term care residents.
• Collaborate with interdisciplinary teams to coordinate and optimize patient care.
• Provide education and support to patients, families, and caregivers on disease management and preventive care.
• Monitor and manage chronic conditions, coordinating with physicians and specialists as needed.
• Participate in regular team meetings and contribute to care process improvements.
• Stay abreast of healthcare trends, regulations, and evidence‑based practices related to long‑term care.
