Job summary

You will collaborate with an interdisciplinary care team, manage complex conditions, and support patients and families in shared decision-making. Our system emphasizes dignity and patient-centered care in a community setting.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Eligible for an unrestricted Maine medical license.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Provide comprehensive medical care to older adults in long-term and other settings.
• Develop individualized care plans based on patient goals and quality-of-life priorities.
• Manage complex chronic illnesses with multiple comorbidities.
• Review medications and optimize regimens for safety and effectiveness.
• Collaborate with nurses, APPs, rehab, specialists, social services, and care team members.
• Support informed decision-making and family engagement in care planning.
• Educate patients on disease management, healthy aging, and preventive care.
• Coordinate care across specialties to ensure continuity.
