Qualifications

• Experience with Epic or other electronic health record systems preferred.

Responsibilities

• Perform comprehensive geriatric assessments including history, exams and functional status.
• Develop individualized care plans based on goals and quality‑of‑life priorities.
• Manage multiple chronic illnesses and polypharmacy in older adults.
• Collaborate with nursing, APPs, rehab, specialists and social services.
• Educate patients and families on disease management and preventive care.
• Coordinate care across settings to ensure continuity and quality.
