Qualifications

• Masters degree or better.
• APRN Advanced Practice RN license.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patients’ response to treatment and adjust plans as needed.
• Work with RN case managers as part of an integrated care team.
• Ensure compliance with local, state, and federal regulations.
