Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with a geriatrics focus to join our team serving residents in nursing homes and assisted living communities enrolled in Medicare Advantage plans.

You will work closely with physicians, case managers, and facility partners to deliver coordinated care, manage acute and chronic conditions, and improve quality of life in a compliant, team-based model.

Qualifications

• Masters degree or higher in nursing or related field
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan
• Prescribe medications and order labs, diagnostics, and consultations
• Monitor patient progress and adjust treatment plans as needed
• Coordinate with RN case managers as part of an integrated care team
