Kalamazoo Long Term Care Pharmacy in Kalamazoo, MI is seeking a Pharmacist with LTC experience to join our team as a Long-Term Care Pharmacist.

As part of our care team, you will provide clinical services, conduct med reviews, oversee dispensing, and ensure regulatory compliance. This role reports to the Pharmacy Manager and offers a full-time schedule with weekday hours and rotating on-call.

Formación

• PharmD from an accredited program.
• Active Michigan pharmacist license or eligibility.
• Long-term care pharmacy experience.
• Clinical geriatrics expertise.
• Strong knowledge of regulations and guidelines.
• Experience with pharmacy software; Framework experience preferred.
• Strong analytical and problem-solving skills with attention to detail.
• Excellent communication and multidisciplinary teamwork.
• Openness to feedback.

Responsabilidades

• Provide clinical pharmacy services tailored to LTC residents.
• Collaborate with physicians, nursing staff, and care team.
• Conduct medication reviews to optimize therapy outcomes.
• Dispense medications accurately and provide clinical oversight.
• Direct pharmacy staff in processing orders and changes.
• Ensure regulatory compliance and LTC best practices.
• Document and communicate using electronic health records.
• Deliver excellent service to patients and caregivers.
• Participate in team meetings to improve resident care.
• Perform other duties as assigned.
