Job summary

Reid Health is seeking a patient-centered NP or PA to join our PACE team, providing primary medical management for participants in collaboration with PACE physicians and the interdisciplinary team.

The APP will focus on comprehensive assessments, ongoing care planning, and supporting the health and well-being of our participants while working 8am to 4:30pm in Pace, Florida. Indiana license is required for this role.

Qualifications

• Successful completion from a qualified NP/PA program.
• Must maintain BLS/ACLS.

• Provide primary medical management for PACE participants in collaboration with PACE physicians and interdisciplinary care team.

Reid Health is seeking a patient-centered and compassionate NP or PA to join our PACE (Program of All-Inclusive Care for the Elderly) team. This APP will provide primary medical management for PACE participants in collaboration with our PACE physicians and interdisciplinary care team, with a focus on comprehensive assessments, ongoing care planning, and supporting the health and well-being of our participants.

Position Details

• Team: 1 physician and 1 NP
• Office Hours: 8am to 4:30pm
