Job summary

Urban Health Plan is seeking a Geriatrician to provide medical care for patients during home visits in the Bronx and in our health centers. The physician will diagnose, treat, and advise on prevention, collaborating with nurse practitioners, PAs, CNMs, and other clinicians while delivering patient-centered care.

The role requires an MD/DO with geriatrics fellowship, board certification, and a valid NY license with CDS/DEA and BLS. Full-time staffing with comprehensive benefits is offered.

Qualifications

Job Description

We employ community-driven and highly skilled clinicians to provide integrated care in our Health Centers. We are seeking new associates to join our team in the capacity of Geriatrician. The Geriatrician will provide medical care to treat, prevent and/or diagnose illness, disease, or injury. The physician will examine or conduct tests to provide information on medical status, analyze records, reports, test results, and/or examination information to diagnose medical condition of patient, prescribe/administer treatment, therapy, medication and/or vaccination. The physician will also focus on explaining procedures and discussing test results on prescribed treatments with patient and advise patients and community concerning diet, activity, hygiene, and disease prevention. The physician will collaborate with and non-physician providers (NP, PA, CNM, etc). The physician will perform other job-related duties as required by departmental needs/demands. Home visits and in office visits.

Experience and Skills

• MD/DO degree from an accredited medical school and completion of accredited residency program


• Completion of Fellowship in Geriatrics


• Board Certified


• Valid NY State license, CDS, DEA, andBLS


• Understanding of and passion for Community Healthcare is preferred
