Job summary

Urban Health Plan is seeking a qualified Geriatrician to provide comprehensive medical care for older adults in our community health centers located in the Bronx, Queens, and Manhattan. The physician will diagnose, treat, and prevent illnesses, and collaborate with other healthcare professionals to deliver coordinated care.

The role requires NY state licensure (CDS/DEA/BLS), Board Certification in Geriatrics, and completion of a geriatrics fellowship.

Qualifications

• MD/DO degree from an accredited medical school and completion of accredited residency program.
• Completion of Fellowship in Geriatrics.
• Board Certified.
• Valid NY State license, CDS, DEA, and BLS.
• Understanding of and passion for Community Healthcare is preferred.

Responsibilities

• Provide geriatric medical care to treat, prevent and diagnose illness.
• Examine patients, analyze records, and prescribe treatments as needed.
• Perform home visits and in-office visits.
• Collaborate with non-physician providers (NP, PA, CNM, etc).

We employ community-driven and highly skilled clinicians to provide integrated care in our Health Centers. We are seeking new associates to join our team in the capacity of Geriatrician. The Geriatrician will provide medical care to treat, prevent and/or diagnose illness, disease, or injury. The physician will examine or conduct tests to provide information on medical status, analyze records, reports, test results, and/or examination information to diagnose medical condition of patient, prescribe/administer treatment, therapy, medication and/or vaccination. The physician will also focus on explaining procedures and discussing test results on prescribed treatments with patient and advise patients and community concerning diet, activity, hygiene, and disease prevention. The physician will collaborate with and non-physician providers (NP, PA, CNM, etc). The physician will perform other job-related duties as required by departmental needs/demands. Home visits and in office visits.
