Job summary

Urban Health Plan is seeking a Geriatrician to provide medical care for patients during home visits in the Bronx and in our health centers. The physician will diagnose, treat, and advise on prevention, collaborating with nurse practitioners, PAs, CNMs, and other clinicians while delivering patient-centered care.

The role requires an MD/DO with geriatrics fellowship, board certification, and a valid NY license with CDS/DEA and BLS. Full-time staffing with comprehensive benefits is offered.

Qualifications

• MD/DO with an accredited residency in geriatrics.
• Fellowship in geriatrics completed.
• Board certification in geriatrics.
• Active NY license with CDS, DEA and BLS.
• Commitment to community healthcare and patient-centered care.

Responsibilities

• Provide medical care during home visits and in-office visits.
• Diagnose, treat, and prevent illnesses for geriatric patients.
• Explain procedures, test results, and treatment plans to patients.
