Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide wound care services to residents living in long-term care facilities. You will diagnose and treat chronic wounds, prescribe medications, and coordinate with an interdisciplinary care team to optimize patient outcomes.

This role requires an unrestricted NP license (KY) and board certification, with experience in wound care and EMR systems. DEA licensure is optional; prior long-term care experience is preferred.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including chronic wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed wounds.
• Familiar with pressure injury staging per NPIAP.
• Experience with wound care products and wound VACs.
• Experience in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide wound care services for long-term care patients.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition.
• Collaborate with the primary care physician.
• Coordinate with the care plan and interdisciplinary team.
