Job summary
CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound care in long-term care facilities. You will manage treatment plans, work with multidisciplinary teams, and coordinate with primary care providers to optimize patient outcomes.
The role requires KY NP licensure, board certification, and experience with wound management and documentation in an EMR system. This is a facility-based position with opportunities for professional growth.
Qualifications
- Active, unrestricted Nurse Practitioner license in Kentucky.
- Board certification in a relevant specialty.
- DEA licensure is optional but preferred for prescribing controlled substances.
Responsibilities
- Provide wound care services for long-term care patients in skilled nursing facilities.
- Prescribe medications, including controlled substances in KY as permitted by law.
- Order, review, and follow up on treatments and diagnostics.
- Communicate with nursing home staff about changes in patient condition and care plans.
- Collaborate with the primary care physician to coordinate care.
