Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to serve patients in long-term care facilities in Kentucky. You will provide wound care services, prescribe medications per state law, order tests, and coordinate with nursing home staff and the primary care physician.

The role emphasizes collaboration within an interdisciplinary team, experience in wound care, and comfort with EMR systems. Part-time schedule offers meaningful patient impact and facility support.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care, including chronic or infected wounds.
• Ability to diagnose and treat diabetic, arterial, venous, and mixed wound etiologies.
• Staging pressure injuries per NPIAP.
• Familiar with wound care products (collagens, alginates, foams, silvers).
• Experience with wound vacs and DME.
• Wound certification preferred or willing to obtain.
• Experience in long-term care.
• Ability to work with an EMR.
• Ability to coordinate care within an interdisciplinary team.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up on treatments and diagnostics.
• Communicate with nursing home staff about changes in condition and care plan.
• Collaborate with the primary care physician.
