Job summary

Catholic Charities Brooklyn and Queens seeks a Care Coordinator to manage day-to-day care coordination for health home members with complex medical and psychiatric needs. You will ensure linkage to medical, behavioral health, and social services, and facilitate timely follow-up and care transitions.

Responsibilities include developing person-centered care plans, coordinating services across providers, and maintaining accurate documentation with focus on reducing ER visits and hospitalizations.

Qualifications

• Bachelor's degree in social work, psychology or related health/human services field.
• Two years of direct work with target population or related experience preferred.

Responsibilities

• Coordinate all aspects of a health home member's care with complex medical/psychiatric needs.
• Facilitate access to medical and psychosocial services and coordinate a plan of care.
