Job summary

HealthForce Healthcare seeks a Registered Nurse (Geriatric) for a hospital travel assignment in Glendale, CA. Start date is 10/12/2026 with a 13-week duration and 36 guaranteed hours per week, day shift from 8:00 AM to 4:30 PM.

The role requires CA RN licensure and BLS certification, with BSN preferred and at least two years of nursing experience, particularly in care management and acute hospital settings.

Qualifications

• RN license in California required.
• BLS certification required.
• BSN preferred.
• At least two years of nursing experience; care management experience preferred.
• Acute care, hospital-based environment experience preferred.
• Case management certification preferred.

Responsibilities

• Coordinate and manage comprehensive care for geriatric patients from pre-admission through post-discharge.
• Lead interdisciplinary care planning to align with patient’s clinical course and needs.
• Collaborate with physicians, nurses, social workers, and care team to optimize outcomes and resource use.
• Monitor diagnostic testing, treatments, and procedures; communicate plan adjustments and discharge planning.
• Focus on complex cases including ED utilizers, chronic pain, substance use, and housing challenges; facilitate transitions of care.
• Support transfers to tertiary centers with required information for continuity of treatment.
