Job summary

Golden Valley Health Centers seeks a PACE Rehab Aide to support the therapy team in delivering rehabilitation care to participants. You will perform nonmedical tasks, such as setting up and cleaning treatment rooms and transporting participants as needed.

Located at Central Valley PACE – Modesto, CA, this full-time role runs Monday–Friday from 8:00am to 5:00pm with guaranteed 40 hours. Benefits include Medical with low deductible, PPO coverages, Dental, Vision, 403(b) with match, FSA plans, and

Qualifications

• Must possess skills to provide care for geriatric participants.
• Excellent oral, written, and computer skills.
• Valid California Driver’s License and insurable driving record.
• Six months experience in professional or personal care setting.
• Current BLS CPR Card (American Heart Association).

Responsibilities

• Assist with participant intake.
• Greet and set up drop-ins on the machines.
• Oversee drop-ins and monitor the gym in absence of licensed staff.
• Transport participants to and from the gym upon request of therapies staff.
• Administrative support of the department, including scheduling of and follow up with participants.
• Observe each participant for changes and report to licensed staff.
• Scheduling and tracking of therapy department logistics.
• Document participants' communication, responses and progress.
• Liaise with physicians' offices and hospital personnel.
• Prepare equipment and supplies; maintain a clean area for treatments and exercises.
• Run and disseminate overdue/missing CPT reports for therapies weekly.
• Daily completion of temperature logs. Assist with therapy treatments on site and in homes.
• Assist activities and day center; maintain participant areas in a clean and orderly manner.
