Job summary

Neighborhood Healthcare PACE is seeking a Rehabilitation Aide to support clinicians in rehabilitative services. This onsite role runs Monday through Friday, 8am–5pm, supporting PT, OT, and SLP teams while assisting with patient mobility, ROM, and ADL activities.

The position emphasizes geriatric care, teamwork, and strong communication. A high school diploma and current BLS certification are required, with CPR/First Aid renewal annually.

Qualifications

• High school/GED required.
• Current Basic Life Support (BLS) certification required upon hire and maintained.
• CPR and First Aid certification required for all PACE employees.
• 1 year of experience with frail and elderly populations; rehab setting preferred.

Responsibilities

• Assist in scheduling, transporting, transferring, and lifting patients for rehabilitation services.
• Document and communicate scheduling conflicts with participants.
• Distribute schedules and information to facility members.
• Maintain rehabilitative equipment and therapy areas.
• Assist PT/OT/SLP with treatments and activities of daily living.
