Job summary

Golden Valley Health Centers is seeking a PACE Activities Specialist for the Modesto center to develop and facilitate recreational programs that support social, emotional, cognitive, and physical well-being of participants. The role involves coordinating group activities, assessments, collaborating with the interdisciplinary team, and ensuring a warm, inclusive environment at the Modesto PACE Center.

Schedule is Monday through Friday, 8:00am to 5:00pm, with benefits including health, retirement,

Qualifications

• Must be able to work effectively in a team environment.
• Good understanding of psychosocial problems of aging.
• Interact professionally and respectfully with geriatric individuals.

Responsibilities

• Conduct initial assessment and reassessment of participants at least every 6 months.
• Develop individual and group therapeutic activity programs with participant input.
• Record progress notes in participant medical records.
