Job summary

Golden Valley Health Centers seeks a Certified Occupational Therapy Assistant to implement therapy programs for PACE patients under the supervision of an Occupational Therapist. The role focuses on helping participants remain independent and live safely in the community.

Work is primarily at the Central Valley PACE Center in Modesto, with additional duties at PACE Merced Center and Modesto McHenry Outpatient Clinic.

Qualifications

• High school diploma or equivalent required.
• Certified Occupational Therapy Assistant in California required.

Responsibilities

• Implements individualized and group occupational therapy treatments under the supervision of the Occupational Therapist.
• Executes therapy programs to restore independence and function for PACE participants.
• Maintains patient records and documents treatments in health records.
• Collaborates with the IDT and care planning to support participant goals.
• Provides services at the Modesto center and at partner locations as needed.
