Job summary

Golden Valley Health Centers is seeking a PACE Activities Specialist to develop and facilitate recreational groups addressing social, emotional, cognitive, and physical needs of participants at the Modesto PACE Center.

The schedule is Mon–Fri, 8:00am–5:00pm, with a pay range of $23.56–$24.73 per hour. Benefits include health/dental/vision, life/AD&D, LTD, retirement plans, flexible spending, and significant vacation/holiday time.

Qualifications

• High School Diploma or equivalent required.
• 36-hour activity leader course preferred.
• Current BLS/CPR certification required.

Responsibilities

• Develops recreational and social activities aligned with plan of care.
• Maintains records and progress notes in participant records.
• Communicates participant changes to the team and updates schedules.
