Job summary

GSSSI is seeking a dedicated Case Manager within the Primary Care Team to serve members enrolled in a Senior Care Option (SCO) plan. The role focuses on coordinating and monitoring contracted and community services to maintain health and independence.

The position is full-time (35 hours/week) with a hybrid schedule (up to 3 days on-site after training). Travel reimbursement is provided; PSLF may apply for eligible not-for-profit employees.

Qualifications

• BA or BS in Social Work, Human Services or related field.
• Experience in case management with geriatric population.
• LSW a plus.
• Spanish/English fluency required.
• Must have valid driver's license and/or reliable transportation.
• Must be able to climb multiple flights of stairs.

Responsibilities

• Function as a case manager within the Primary Care Team for SCO participants.
• Make recommendations, coordinate, implement, and monitor services.
• Ensure access to public benefits to support wellness and independence.
• Conduct periodic reassessments to determine appropriateness of the service plan and prevent premature institutionalization.

Responsible for functioning as a case manager within the Primary Care Team (PCT) for members participating in a Senior Care Option (SCO) plan. Make recommendations, coordinate, implement, and monitor contracted and community services for the purpose of improving or maintaining the member's overall health and functional status. Conduct periodic reassessments to determine appropriateness of the service plan and ensure that members have access to public benefits to support their wellness, independence, with the goal of preventing premature institutionalization.

Travel reimbursement is $0.725/mile

Minimum Qualifications: BA or BS in Social Work, Human Services or related field, experience in case management with geriatric population. LSW a plus. Must have valid driver's license and/or reliable transportation. Spanish/English fluency required. Must be able to climb multiple flights of stairs.
