AgeSpan is seeking a Geriatric Support Services Coordinator (GSSC) to assess elders, develop and monitor service plans, and coordinate care with an interdisciplinary team in the member's home.

The Geriatric Support Services Coordinator (GSSC) will conduct health and functional assessment for elders enrolled in Senior Care Organizations (SCO). The GSSC will also develop and monitor service plans to promote the client's overall health and independence. The GSSC will work with an interdisciplinary team to assure effective coordination and delivery of care in the member's home.
(Core duties or tasks that are fundamental and not marginal to the performance of the job
BA/BS in social work, human services or related field plus experience with Elders in a Health Care setting required. Associates Degree with significant relevant work experience may be substituted for a portion of degree. Strong written, verbal, and computer skills necessary to work successfully in an interdisciplinary team required. Must have valid Driver's License and reliable transportation. A bilingual capacity in English along with either Spanish or Khmer is required.
At AgeSpan, we carefully consider a wide range of factors when determining rate of pay. Actual rate of pay depends on factors including but not limited to internal equity, experience, and qualifications. The pay range for this position is $30.00 - $31.50/hour.
