Job summary

Alta-Heights-Post-Acute in Nashville, TN seeks a licensed Speech-Language Pathologist to evaluate residents, develop and refine treatment plans, and provide therapy to improve swallowing, cognition, and communication. The role includes educating families and collaborating with physicians and nursing staff.

You will supervise CFY trainees and students, participate in care conferences, and lead staff in-services, ensuring compliant documentation and quality care within facility policies.

Qualifications

• Licensed Speech-Language Pathologist in the state with active license and CE requirements.
• Proficient in documenting care per facility and payer rules.
• Ability to supervise CFY or students as applicable.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Educate residents, families, and caregivers on goals and home programs.
• Communicate progress and discharge plans with an interdisciplinary team.
• Provide in-services and training for facility staff.
• Supervise CFY- Speech Clinical Fellows and students as applicable.
