Job summary

Union City Health & Rehabilitation in Union City, TN, is seeking a dedicated PRN Speech-Language Pathologist to join the in-house rehab program and serve geriatric patients. The role involves evaluating patients, delivering skilled therapies, and coordinating care within a post-acute setting.

A strong team environment supports professional growth and patient outcomes. Licensed SLP required; new grads welcome.

Qualifications

Speech-Language Pathologist license is required.
Open to SLPs at all experience levels. New Grads are welcome!

Responsibilities

Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, creating appropriate care plans.
Deliver rehabilitation, skilled and medically necessary treatment interventions.
Assist with standing, walking, lifting, transferring and repositioning of patients as needed.
Administer procedures as part of the rehab plan, including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
Provide consultation to patients, families, caregivers, and other service providers.
Generate discharge summaries ensuring regulatory and clinical compliance.
