• Evaluate residents per MD order and establish treatment plans and goals.
• Reassess caseload every 30 days to determine continuation or discontinuation of services.
• Provide discharge planning to maximize independence in the least restrictive environment.
• Educate staff, family, and caregivers on treatment plans and progress.
• Coordinate in-services for facility staff to ensure safe carryover of programs.
• Maintain documentation per guidelines and comply with regulations.
