Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care in homes and senior living communities in South Lyon, MI. You’ll conduct evaluations, create individualized plans, and deliver evidence-based therapy to improve mobility and independence.

The role offers autonomy over scheduling, the possibility of telehealth, and the chance to build lasting patient relationships within a clinician-driven, home-based care model.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
