Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living care. You will evaluate patients, create individualized plans, and deliver evidence-based therapy to improve safety, function, and independence.

Documentation in FOX's EMR and collaboration with PTs, OTs, SLPs, and Fitness Specialists are required. Flexibility in scheduling, opportunities for telehealth, and a clinician-driven culture support professional growth and meaningful patient

Qualifications

• Valid Speech Language Pathology license in state(s) of practice.
• EMR proficiency and basic computer skills.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy to promote safety, function, and aging in place.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and independence.
• Provide telehealth / virtual care when appropriate.
