Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in homes and senior living settings in West Bloomfield, MI. You’ll create and manage treatment plans, collaborate with an interdisciplinary team, and leverage telehealth to extend access and continuity of care.

Qualifications

• Licensed Speech-Language Pathologist in MI or eligible to obtain
• Degree from an accredited Speech Language Pathology program
• Current CPR certification
• Current ASHA CCC or eligibility for CCC

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate
