Job summary

FOX Rehabilitation in Ann Arbor, MI is seeking a licensed Speech Language Pathologist to deliver one-on-one in-home and senior living care. You’ll conduct evaluations, develop and deliver individualized plans of care, and document outcomes in FOX’s EMR.

The role offers autonomy to manage your own treatment schedule and the option to incorporate telehealth when appropriate. You’ll collaborate with an interdisciplinary team, educate patients and caregivers, and contribute to a mission-driven

Qualifications

• Valid Speech Language Pathology license in the state of practice or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current ASHA CCC (if applicable).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in-home or in senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
