Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care in Ann Arbor, MI, delivering services in private residences and senior living communities as part of the Geriatric House Calls model.

You will evaluate, treat, and document patient progress, collaborate with an interdisciplinary team, and educate patients and caregivers to promote safety and aging in place, including telehealth options when appropriate.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited SLP program.
• Current Certificate of Clinical Competence (ASHA CCC).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or communities (private residences, senior living).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
