Job summary

Almost Heaven Nursing Center is seeking a licensed social worker to assess psychosocial, mental, and emotional needs of residents and coordinate access to services.

The role requires state licensure, a bachelor's degree in social work or related field, and experience in geriatric healthcare; the social worker will maintain documentation and collaborate with the care team to support resident rights and transitions.

Qualifications

• Licensed, certified, or registered in the state of practice is required.
• Bachelor’s Degree in Social Work or in a related Human Services field from an accredited school.
• At least 1 year of supervised social work experience with geriatric residents in a healthcare setting.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Complete social history and psychosocial assessment for each resident and guide the care plan.
• Maintain accurate documentation in line with regulations and facility policies.
• Support residents' self-determination and educate on alternatives.
• Participate in interdisciplinary care planning and discharge planning.
• Provide psychosocial support to residents and families; coordinate referrals.
• Collaborate with nursing and administration to prepare for inspections.
• Assist in ongoing social services program planning and evaluation.
• Other duties as assigned.
