Job summary

Providence PACE is seeking a per diem on-call Social Worker who will provide direct social work services to elderly participants in the Seattle area. The role involves counseling, assessments, and coordinating with interdisciplinary team to create individualized plans of care.

The successful candidate will hold a Master's in Social Work and applicable Washington licenses, with at least one year of geriatric experience and two years working with diverse cultures.

Qualifications

• Master's Degree in Social Work required.
• Licensed clinical social worker credentials required or must be obtainable on hire.
• Experience with geriatric populations and diverse cultures preferred.

Responsibilities

• Provide social work services including one-on-one and group counseling.
• Assess social, emotional, cognitive and ethnic factors to create integrated care plans.
• Access community resources and ensure continuity of care across settings.
• Conduct social work assessments in the participant's home as needed.
