Qualifications

• Master's Degree in Social Work.
• Upon hire: Washington Clinical Independent Social Worker Associate License or Temporary License or
• Upon hire: Washington Clinical Independent Social Worker License or
• Upon hire: Washington Clinical Independent Social Worker Temporary License.
• 2 years Social Work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services to participants and support direct service activities within the Center.
• Conduct comprehensive social work assessments and develop individualized care plans.
• Access community resources and ensure continuity of care across the continuum.
• Perform social work interventions in homes and other settings as needed.
