Providence PACE is a Program of All-Inclusive Care for the Elderly that helps keep seniors healthy at home through clinics and home visits. As a social worker on the interdisciplinary team, you will deliver direct services, perform assessments, and help create individualized care plans.

Qualifications

• Master's Degree in Social Work required.
• Washington licenses required on hire (independent social worker or temporary).
• 2 years of social work with geriatric populations from diverse ethnic cultures.
• 1 year of work experience with geriatrics.

Responsibilities

• Provide social work services in direct or group formats within the Center.
• Assess and develop integrated care plans considering social, emotional and cognitive factors.
• Access community resources and ensure continuity of care across settings.
• Collaborate with interdisciplinary team and support home visits as needed.
