AltaMed Health Services Corp. is seeking a Social Worker to support the PACE program with psychosocial assessment, care planning, and discharge planning.

The MSW qualification guides a role focused on interdisciplinary collaboration and resource mobilization for older adults and their families. Responsibilities include case management, advocacy, and addressing behavioral health needs within a holistic, person-centered framework.

Qualifications

• Minimum of 2 years of experience in case management, social advocacy, and/or mental health, with the geriatric population.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide psychosocial assessment, care planning, coordination, and discharge planning for PACE participants.
• Offer individual and group interventions, education, and advocacy leveraging social determinants of health.
