Job summary

St. Paul’s Senior Services in California is seeking a full-time Social Worker for the PACE Social Work department to plan, organize, and implement comprehensive social work services for participants and families.

Under supervision, the role includes education, assessment, counseling, resource provision, ongoing case management, and advocacy to meet participant and caregiver needs. Typical hours are Monday through Friday, 8:00 am to 4:30 pm.

Qualifications

• Minimum of one year experience working with frail or elderly populations.
• Experience on a multidisciplinary team in hospital, nursing home, or community setting preferred.
• Training or related experience in the job assigned.

Responsibilities

• Plan, organize, and implement social work services for PACE participants and families.
• Provide psycho-social assessment, counseling, and education to participants and families.
• Coordinate resources and ongoing case management for participants.
• Advocate to ensure participant and caregiver needs are met and address enrollment/disenrollment procedures.
• Liaise between the interdisciplinary team, caregiver representatives, and community agencies.
