Job summary

St Paul's Senior Services in San Diego, CA, is seeking a full-time Social Worker for the PACE program. Under the Social Work Manager, you will plan, organize and implement services for participants and families, including education, assessment, counseling, resources and ongoing case management.

You will liaise with the interdisciplinary team, caregiver representatives and community agencies to address needs and coordinate enrollment and disenrollment procedures.

Qualifications

• Master’s Degree from an accredited university in Social Work.
• Minimum one year of experience with frail or elderly populations.
• Experience on a multidisciplinary team in hospital, nursing home, or community setting.

Responsibilities

• Plan, organize, and implement social work services for PACE participants and families.
• Provide psycho-social assessment, counseling, and resources; ongoing case management.
• Advocate to ensure participant and caregiver needs are met; address mental health needs.
• Serve as liaison between the interdisciplinary team, caregiver representatives, and community agencies.
• Support enrollment and disenrollment procedures as appropriate.
