Talent Connect Executive Search & HR Solutions is recruiting a Social Worker for a PACE program. The role provides psychosocial support, care coordination, and advocacy for older adults with complex needs within an interdisciplinary team and community-based settings.

The Social Worker will assess, plan, and connect participants with resources, support families, and help navigate healthcare and social services while maintaining thorough documentation and ensuring safety.

Qualifications

• MSW from an accredited university.
• Strong psychosocial assessment, care planning, and resource coordination.
• Ability to support older adults and families navigating complex healthcare and social service needs.
• Strong communication, documentation, and problem-solving skills.
• Ability to collaborate effectively with a multidisciplinary healthcare team.
• Commitment to respectful, culturally responsive, participant-centered care.

Responsabilités

• Complete initial, periodic, and change-in-condition psychosocial assessments.
• Develop and implement individualized social work interventions.
• Provide supportive counseling and education to participants and caregivers.
• Connect participants with community resources and benefits programs.
• Participate in IDT meetings and communicate psychosocial concerns.
• Collaborate with clinical staff to support care transitions.
• Facilitate advance care planning discussions and connect to resources.
• Assist with discharge or disenrollment coordination.
• Recognize and respond to abuse, neglect, exploitation, or safety concerns.
• Maintain accurate documentation of assessments, interventions, and outcomes.
• Coordinate referrals for specialized clinical support.
