Qualifications

• Bachelor’s degree in social work required.
• Experience in Long Term Care/Subacute setting is a plus.
• Demonstrated competence in management and leadership skills.
• Knowledge of state and federal regulations.

Responsibilities

• Maintain and improve residents' ability to manage daily physical, mental, and psychosocial needs.
• Interview residents and families to identify social and emotional needs.
• Develop and revise care plans with goals for each need.
• Participate in interdisciplinary care plan meetings and discharge planning.
• Refer residents to social services when facility services are unavailable.
• Provide information on resident rights and finances.
