Job summary

Bethesda Health Group is seeking a Social Worker to support residents in a long-term care setting. You will perform social assessments, coordinate care plans with an interdisciplinary team, and ensure documentation and charting are completed accurately.

You will collaborate with residents, families, and representatives, assist with Medicaid applications, and arrange appointments and transportation as needed to meet residents’ social service needs.

Qualifications

• MSW with geriatrics focus preferred
• Strong documentation and charting abilities required
• Ability to understand resident rights and advance directives
• Experience in long-term care social service department preferred

Responsibilities

• Provides goal-oriented group and/or person-centered social sessions for residents
• Addresses resident and family issues; serves as a resource for adjustments to long-term care
• Participates in Resident Care Plan meetings and interdisciplinary team discussions
• Completes required documentation and maintains charting; updates advance directives annually
• Assists with Medicaid applications and scheduling appointments; coordinates transportation as needed
• Engages with residents and families to develop care plans and address concerns in writing
