Job summary

Bethesda is seeking a Social Services Coordinator to evaluate and address residents' social service needs, develop relationships with residents and families, and ensure timely support.

The role includes initiating charting, developing activity programs, monitoring progress quarterly, updating resident rights, and participating in Resident Care Plan meetings. Bachelor’s degree in social work is required; geriatric experience preferred, with strong organizational and communication skills.

Qualifications

• Bachelor's degree in social work required.
• Knowledge of growth and development and geriatric population.
• Two years experience preferred.
• Geriatric and Social Work experience preferred.
• Excellent organizational and communication skills.
• Ability to perform documentation in writing and electronically.
• Manual dexterity and skillful use of hands when working with patients and equipment.

Responsibilities

• Evaluate and assess residents' social service needs and concerns.
• Develop relationships with residents and families and ensure timely support.
• Initiate social service charting and activity program development upon admission.
• Monitor residents quarterly and complete required charting.
• Participate in Resident Care Plan meetings.
• Complete quarterly progress notes and social service log.
