Carease Health - IL is seeking a Social Worker/Care Companion to provide emotional support and short-term therapy for residents in skilled nursing facilities. The role involves coordinating with the care team and psychiatric providers to ensure comprehensive, resident-centered care.

Responsibilities include assessments, brief therapeutic interventions, and thorough documentation. Flexible part-time schedule (~21 hours/week) with paid training and ongoing clinical support from our team.
