Job summary

Carease Health is seeking a compassionate Care Companion/Social Worker to join our Collaborative Care team, delivering emotional support and brief therapy to residents in skilled nursing facilities.

You will visit facilities, assess residents’ needs, document notes, and collaborate with psychiatric professionals, including a monthly meeting with the psychiatric NP. The role offers a flat salary and opportunities for paid training and growth.

Qualifications

• A degree in Social Work, Psychology, Counseling, or related mental health field.
• Understanding of geriatric mental health and crisis management in long-term care.
• 1 year of experience in mental health or social work.

Responsibilities

• Visit designated facilities to assess residents’ emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records after each visit.
• Collaborate with psychiatric professionals, including monthly meetings with the psychiatric NP.
