Job summary

Carease Health - NJ seeks a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker. You will support residents in skilled nursing facilities with emotional care, brief therapy, and mental health assessments.

This on-site role offers regular supervision, monthly clinical meetings, and opportunities for growth while helping older adults feel heard, supported, and emotionally connected in their daily environment.

Qualifications

• Degree in social work, psychology, counseling or related mental health field.
• Understanding of geriatric mental health and crisis management in long-term care.
• Strong communication and relationship-building abilities.
• Minimum 1 year of mental health care experience.

Responsibilities

• Visit designated facilities as scheduled
• Assess residents' emotional, cognitive, and psychosocial needs
• Provide short-term, solution-focused therapy and supportive interventions
• Document visit notes and maintain records
• Collaborate with psychiatric professionals in monthly meetings
