Job summary

Carease Health seeks a part-time Social Work/Care Companion to provide emotional support, brief therapy, and mental health assessments for residents in skilled nursing facilities. You will work with our collaborative care team to deliver care focused on both physical and mental well-being.

The role emphasizes geriatric mental health, crisis support, and thorough documentation, with participation in monthly meetings with the psychiatric NP.

Qualifications

• Associates, Bachelor’s, or Master’s degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree.
• Experience: 1 year.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
