Job summary

Carease Health - OH is seeking a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker in a part-time role serving residents in skilled nursing facilities.

You will provide emotional support, brief therapeutic interventions, mental health assessments, and collaborate with psychiatric professionals, including monthly NP meetings. Flexible scheduling and paid training are offered to support professional growth.

Qualifications

• Strong communication and documentation skills.
• Geriatric mental health knowledge and crisis management.
• Experience in long-term care settings preferred.

Responsibilities

• Visit designated facilities to assess residents' emotional, cognitive, and psychosocial needs.
• Conduct assessments and provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records of interactions and interventions.
• Collaborate with psychiatric professionals, including monthly meetings with the psychiatric NP.
