Carease Health in Cleveland, OH is seeking a Social Worker/Care Companion to join our Collaborative Care team. You will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

The role involves visiting facilities, conducting psychosocial assessments, delivering short-term therapy, documenting interactions, and coordinating with psychiatric professionals, including monthly meetings with the psychiatric NP.

Formación

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health and crisis management.
• Strong communication, documentation, and relationship-building skills.
• At least 1 year of experience in a similar role.

Responsabilidades

• Visit designated facilities per schedule with supervisor.
• Assess residents' emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records.
• Collaborate with psychiatric professionals and attend monthly meetings with the psychiatric NP.
