Carease Health is seeking a compassionate mental health provider to join our Collaborative Care team in skilled nursing facilities. You will offer emotional support, brief therapy, and mental health assessments to older adults, helping them feel heard and connected within their care environment.

The role emphasizes direct resident interaction, supervision, and collaboration with psychiatric providers to ensure comprehensive care and ongoing professional growth through monthly clinical meetings

Qualifications

• Experience in geriatric mental health and long-term care settings.
• 1 year of experience in mental health field.
• Strong communication and documentation skills.

Responsibilities

• Visit designated facilities according to schedule with supervisor.
• Assess residents to identify emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records after each visit.
• Collaborate with psychiatric professionals and participate in monthly meetings with the psychiatric NP.
