Qualifications

• LSW licensure and a strong interest in geriatrics.
• At least 1 year direct social work experience in long-term care.
• Experience counseling and advocating for patients and families.

Responsibilities

• Develop a comprehensive social history for each patient.
• Provide social interventions and counseling as a patient advocate and liaison with the community.
• Maintain progress notes reflecting psycho-social and family dynamics and plan progress.
• Develop initial discharge plan and document in clinical records, revising with community services.
