Job summary

Blaire House of Worcester seeks an energetic Licensed Social Worker to uphold high standards of senior care. You will develop social histories, provide counseling as a patient advocate, and coordinate with families and community resources.

The role requires a solid background in geriatric social work, licensure at LSW level or higher, and at least one year of long-term care experience. Join a caring team delivering quality elder care.

Qualifications

Responsibilities

• Develop a comprehensive social history for each patient.
• Provide social interventions and counseling as a patient advocate and liaison with the community.
• Maintain progress notes reflecting psycho-social and family dynamics and plan progress.
• Develop initial discharge plan and document in clinical records, revising with community services.

Job Description

• Develop a comprehensive social history.
• Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues.
• Maintaining social service progress notes on patient medical records which reflect any changes in patient psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress made.
• Develop initial discharge plan and document in clinical record, revising until discharge with available community services.

Qualifications

• EDUCATION: B.A. or M.A. degree in social work or related field.
• Must be a licensed social worker at the LSW level of licensure or higher.
• TRAINING AND EXPERIENCE: At least one year direct social work experience in a long term care facility.
• Must be a licensed social worker at the LSW level of licensure or higher.
• Must have a genuine interest in working with geriatric patients and a sincere concern for their welfare and happiness.
